| Term | Details |

| Retainer | Base fee (IME / CME or Record Review Only) plus applicable travel fee is due within 14 days of scheduling to confirm the exam date. |
| Medical Records | All records must be shared at least 21 days in advance of exam date or report deadline and will otherwise be subject to the Expedite Fee. Uploading records as soon as possible is greatly appreciated. |
| Final Report Release | All outstanding balances must be paid before delivery of the final report. |
| Cancellations / Refunds | Full refund if canceled ≥3 weeks before the exam; non-refundable within 3 weeks. Within 24 hours of booking your exam, we are happy to cancel or reschedule at no charge (regardless of exam date). |
| Rescheduling | No charge if rescheduled ≥3 weeks before the exam; within 3 weeks, only an additional exam fee will apply (i.e. no additional record review, travel, or other fees). No-shows will require full Retainer payment to rebook. |
| Late Payments | Invoices unpaid after 30 days accrue 1.5% monthly interest; services may be suspended until payment is received. |
| Accepted Payment Methods | Secure online payment (ACH or wire) is strongly preferred; checks payable to CaseLynx, Inc. are also accepted. |
